Navigation Services
The Patient Navigator program helps patients, families, and caregivers navigate the thru the many systems needed during the cancer journey. Trained Patient Navigators link those dealing with cancer to needed programs and resources.
Patient navigator program is available to help patients and their families find and make use of the various resources, treatment services, and support services that they need to receive the best cancer treatment possible.
Once a cancer diagnosis has been made, an assessment and inventory of the patient’s needs and concerns is used to determine an individualized care plan for the patient.
The patient navigation service is a program that:
Highly trained, caring individuals, who are culturally sensitive to the unique needs of Hawaii’s cancer patients. We believe that cancer patients and their families should not have to navigate their cancer journey alone.
Patient navigator program is available to help patients and their families find and make use of the various resources, treatment services, and support services that they need to receive the best cancer treatment possible.
Once a cancer diagnosis has been made, an assessment and inventory of the patient’s needs and concerns is used to determine an individualized care plan for the patient.
The patient navigation service is a program that:
- helps patients navigate through the maze of necessary medical appointments
- works collaboratively to promote communication between patients and healthcare providers by reinforcing physician-patient relationships
- coordinates patient’s services throughout the continuum of breast care and treatment including post-therapy coordination of appointments for ongoing surveillance and rehabilitation
- connects patients with outside community resources
- assists uninsured patients to acquire medical insurance through the State’s Medicaid program; and/or apply for the financial discount program available at QMC
- coordinates and provides screening, diagnostic mammograms and therapy for the underserved population via the nationally funded Center for Disease Control (CDC) and Hawaii State Department of Health’s Breast Cervical Cancer Control Program (BCCCP)
